I voluntarily give my consent to the provision and processing of personal data according to EU Regulation 679/2016 to the extent of the data contained in this form, i.e. in particular first name name, last name, e-mail address and phone number of the administrator, i.e. Effective online s.r.o. with its registered office at Ve Střešovičkách 2486/70, Prague 169 00 - Břevnov (hereinafter referred to as the "Administrator"). I provide my consent for the purpose of processing the offer of requested services at Effective online group, i.e. providing information by e-mail or phone. Sending commercial communications by electronic means according to Act No. 480/2004 Coll., on Certain Information Society Services, as amended.

Personal data will be processed manually and automatically by the administrator. I acknowledge that the administrator is obliged, at my request, to provide me with information about the processing of my personal data, its purpose, the personal data that are the subject of processing, including information about their source, the nature of any automated processing and the recipients of personal data, i.e. those to whom my personal data have been disclosed. I also acknowledge that I have the right to request from the administrator an explanation of the processing of my personal data in the event that I find out or believe that this processing is not carried out in a proper manner, as well as the right to request the administrator to remedy this situation, consisting, in particular, in blocking, correcting, supplementing or deleting this data. I give my consent voluntarily and I acknowledge that I have the right to withdraw it at any time. I give this consent for a period of 60 months from its granting.
